Local authorities across the Diocese of Chester are working in partnership to support individuals and households who may be vulnerable during this period of restricted travel and social distancing.
In this case, vulnerability does not necessarily need to be formally classified and can range from those who require assistance in obtaining food shopping and medical prescriptions, to those who are feeling isolated and need someone to check in on them via the phone to have a chat.
Links are made with a named key worker who will provide support to meet the needs of the individual and who will work alongside other statutory and voluntary organisations to signpost the individual to other services.
